Follow-up care and rehabilitation

Facilitating patients’ return to normal life

We support public institutions and groups of clinics or hospitals in the creation and launch of inpatient or outpatient follow-up care centers and the implementation of an organization adapted to the territory to facilitate the return to normal life of patients while ensuring a smooth and safe transition to an adapted resumption of activity.

Presentation

The importance of rehabilitation centres in improving the health of the population is no longer to be demonstrated.

Being able to quickly return to normal activity or gain independence and autonomy after surgery or a health problem (cardiac or neurological for example) is critical and requires specific, multidisciplinary support and adapted equipment. However, the development of follow-up care centres remains very insufficient in many countries.

Analysis

The follow-up care and rehabilitation sector is facing several major challenges, including:

  1. Inequitable access to follow-up care and rehabilitation facilities in the territory and an ever-increasing demand due to the ageing of the population and the increase in chronic diseases.
  2. Adaptation to the specific needs of patients due to the diversity of pathologies that require specialized and personalized care.
  3. Coordination between follow-up care and rehabilitation, hospitals, general practitioners and home services in order to avoid breaks in care pathways.
